Immunization Requirements PreK-12

 

Grand County School District complies with state law and Utah Department of Health recommendations for childhood vaccinations for school entry. School staff may access the Utah State Immunization Information System (USIIS) for student immunization records, but it is the parent's responsibility to provide complete immunization records upon the child's enrollment to school.

Immunization Exemptions

 

Immunization exemptions are allowed per Utah law for personal, medical or religious reasons with completion of a vaccination exemption form and a notice from a licensed physician (for medical exemption). The exemption form can be obtained through completion of the online education module and returning the certificate of completion to school staff. Exemption forms are valid when a student changes schools, but must be renewed when:

 

    • the student enrolls in Kindergarten, or when the child turns six
    • the student enrolls in 7th grade, or when the child turns twelve
 

 

Chickenpox Verification
 
Proof of immunity to a disease can be accepted in lieu of vaccination with documentation from a healthcare provider that the student has prior history of the disease.
Immunization Records
 
You may access your immunization records if you are 18 years or older, or the legal parent/guardian of a child under 18. Because immunizations are considered a confidential record, you may be required to verify your identity and/or sign a release.
 
You may access your children's immunization records:
  1. From your healthcare provider: most providers are connected to the Utah Statewide Immunization Information System (USIIS) and can provide a USIIS record or a personal immunization record.
  2. Through the Docket Mobile app
  3. From the Utah Department of Health's Utah Statewide Immunization Information System (USIIS)
  4. From the Southeast Utah District Health Department: complete a medical release and identification and bring it to 575 Kane Creek Blvd.
